Adoption Paperwork Summary
Solas requires two sets of adoption paperwork. One goes with the adopter and one goes back to Solas (after adoption) for recording.
1. Adoption Contract (2 copies)
The adoption contract can be printed and filled out by hand or created out of the same app that you requested a file number and the intake form.

2. Rabies Certificate (or voucher if not old enough)
-The original (with the tag) goes with the adopter.
-A copy of the certificate must be attached to the Solas paperwork (or submitted digitally)
**If voucher is needed and you are creating the contract via the app, be sure to check the “voucher needed” box and it will automatically create one for you.
3.Vet Records/Temp Home Records
These documents are given to the adopter. Solas does NOT need a copy of these because the important information for Solas to have such as vaccination information is already on the contract.
4. Microchip Information
All cats must be microchipped prior to adoption. Solas Recue uses Petlink microchips. Some cats may come in already chipped, in which case we need the number and vendor (if possible). PLEASE note on the Solas copy of the contract that it’s not a Petlink chip.
Currently, Valley View Vet and Completely Cat Clinic keep a stock of OUR microchips and implant them for free at time of spay/neuter. (Please remember to request this when checking in)
If not done by the vet, please reach out to Sarah or foster mentor to get this completed. We have many volunteers who are comfortable doing this.
AT TIME OF ADOPTION: Original microchip card/tag goes to the adopter. They also need a copy of our microchip tag information.
Optional
Adoption Application -If you had them fill out a printed adoption application, it goes with the Solas paperwork.
ICat intoductions – Unless you KNOW they do not have other pets or are experts, a copy of “how to introduce a new cat” is advisable
After the adoption
If the foster is adopted from Humane Pet Services, they collect the paperwork and payment for Sarah/Jackie to pick up.
If you adopt out of your home, you are responsible for getting paperwork to Sarah/Jackie.
ALL CASH adoptions must be turned in by hand.
CC or E-Pay adoptions, you may scan the completed paperwork and submit via the app
